Creekview Elementary School Clinic
The CES Clinic is open from 8:10 am - 4:10 pm. If you need to meet with Nurse Bennett before or after clinic hours, please contact via phone or email to schedule a meeting.
Parents, please remember that all medications should be brought to the CES clinic by an adult. Please DO NOT send medications with your student to school. All medications brought by a student will be held in clinic until an adult can sign them out. Any unclaimed medication will be disposed of at the end of the school year.
All Prescription medications given at school will need a physician signature on the medication permission form, in addition to a parent signature. No medication will be given without the proper paperwork. Please be prepared and ask questions if needed.
Students with a food allergy will require a Food Allergy Action Plan to be completed by the doctor.
Students with asthma will require an Asthma Action Plan to be completed by the doctor. These forms are attached below.

- 24 hours fever free without the use of fever reducing medication.

- 10 days have passed since the onset of symptoms (you may obtain a doctor's note with an alternative diagnosis to explain symptoms and their clearance to return to school)
